There is only one "you," but depending on specific circumstances — like if you get married, divorced, remarried or decide to legally change your name — your records may not be properly updated. And it's up to you to ensure that those records are accurate.

This letter is my formal request that you change your records to reflect my new legal name. In addition to my new name, please update my records to indicate my new address. Below I have listed, along with my Social Security number, my prior name and previous address and my new name and address to which my records should be changed:
